Supported Living Services

Overview

Disabled adults, 22 years or older, who have the capacity to live independently in their own homes or apartments can do so successfully through this program. Supported living may be initiated up to 90 days prior to moving into the home of an individual’s choosing.

Housing acquisition, roommate selection (if desired), budgeting, meal planning and preparation, financial management, cooking, cleaning, shopping, personal care, maintaining personal relationships and community connections, coordination of medical services are among the many benefits that supported living provides.

Clients have access to staff 24 hours per day, seven days a week, in case of emergencies. 


Assessments

To identify the types of training, assistance, and intensity of support needed for the client, ICAN, Inc. will complete a formal implementation plan, also referred to as an (IPP), developed under the direction of the client, reflecting their goal(s) from the current support plan.

ICAN, Inc. will also complete a Functional Community Assessment Form. The assessment addresses all aspects of daily life. The assessment addresses relationships, medical and health concerns, personal care needs, household and money management, community mobility, and community interests.

A housing survey is also reviewed quarterly as part of the quarterly home visit and made available for review by the WSC. This survey also includes a review of the recipient’s overall health, safety, and well-being.
